Application Requirements

To be considered for admission to St. Mary of the Hills School, all students must have a completed application packet on file.  The application packet consists of:

 

  • Copy of Birth Certificate

  • Copy of Catholic Baptismal Certificate (if applicable)

  • Copy of Report Cards (full record from last completed school year & any progress reports/report cards from the current school year)

  • Recommendation Form to be completed by current teacher (select the appropriate form):

Form for students entering Pre-Kindergarten
Form for students entering Kindergarten
Form for students entering Grade 1
Form for students entering Grades 2-8

  • $50 Testing/Screening Fee (checks payable to St. Mary of the Hills School). This fee is non-refundable. After school has closed for the summer, the Testing/Screening fee is $75.

 Once all required forms have been submitted, the applicant will be given a screening or testing date.
